1. This Instruction was developed in order to implement the Law of the Republic of Bashkortostan "On state awards and honorary titles of the Republic of Bashkortostan" and the Regulations "On the medal "Maternal Glory", approved by the Resolution of the State Assembly of the Republic of Bashkortostan dated March 5, 1998 N GS-384.

The instruction defines the procedure for presenting mothers of large families for the awarding of the Mother's Glory medal.

2. The medal "Maternal Glory" is awarded to mothers who gave birth (adopted, adopted) and raised (bringing) five or more children, provided that one of them was born after April 16, 1997, residing (registered) on the territory of the Republic of Bashkortostan.

Mothers of many children who conscientiously and responsibly relate to the upbringing of their children, lead a healthy lifestyle, ensure the proper level of care for the health, education, physical, spiritual and moral development of children, the harmonious development of their personality, setting an example in strengthening the institution of the family and raising children.

b) copies of birth certificates of children;

c) copies of certificates of adoption (adoption) - for adopted (adopted) children;

d) documents (information) about persons living together with the applicant;

e) references military units or military commissariats - for children in military service;

f) certificates of military units or military registration and enlistment offices or other documents on the death (death) of children - for children who are missing or died under the circumstances specified in paragraph four of clause 3 of these Instructions;

g) certificates from the place of study, work of children about the fact of study, work - for children living separately from their mother;

h) characteristics of the mother of many children and her family from the applicant;

i) information (information) from the educational authorities, guardianship and guardianship, internal affairs and the commission on juvenile affairs about the conscientious and responsible attitude of the mother of many children to the upbringing of children (about the absence of cases of improper performance by the mother of the duties of raising children);

j) taxpayer identification number (TIN);

k) a copy of the compulsory pension insurance certificate (SNILS);

l) a document confirming the change in the data of the mother, children (a copy of the certificate of marriage or divorce, a copy of the certificate of name change, a certificate from the civil registry office, a court decision).

Copies of the documents referred to in this clause shall be compared with the originals and certified in accordance with the established procedure.

The applicant party requests the necessary documents (information) at the disposal of state bodies, local government bodies, organizations subordinate to them, if these documents are not presented by the mother of many children personally.

A mother of many children has the right to submit all the necessary documents specified in this paragraph, in in full on their own initiative.

The documents specified in this paragraph shall be considered valid for a year from the date of sending the proposal to initiate a petition, provided that the date of issue of the documents (certification of their copies in the prescribed manner) does not exceed one month before the date of sending the proposal to initiate a petition.

5. The administration of the municipal district (urban district), on the basis of the proposal made in accordance with paragraph 4 of these Instructions, within a month from the date of its receipt, checks the submitted documents and, if a positive decision is made, prepares an application for the presentation of a mother of many children for awarding the medal "Maternal Glory" "in the form of a resolution of the administration of the municipal district (urban district) in the form in accordance with Appendix No. 1 to this Instruction, submits this petition to the Ministry of Labor and Social Protection of the Population of the Republic of Bashkortostan through a subordinate institution authorized by it (hereinafter referred to as the institution of the Ministry of Labor of the Republic of Belarus).

Copies of the documents specified in subparagraphs "a - c", "h", "k - m" of paragraph 4 of this Instruction, certified in the prescribed manner by the administration of the municipal district (urban district), are attached to the application.

The documents specified in subparagraphs "b" and "m" of paragraph 4 of this Instruction are submitted in cases of inconsistency in the submitted documents with the surname of the mother of many children presented for awarding with the surname of the children (child).

6. Grounds for refusal to submit a petition for awarding a mother of many children with the medal "Maternal Glory":

a) non-compliance with the criteria established by paragraphs 2 and 3 of these Instructions;

b) a negative characterization of the personality, information (information) of the educational authorities, guardianship and guardianship, internal affairs and the commission on minors' affairs about the improper performance by the mother of many children of the duties of raising children;

c) the absence of the documents specified in paragraph 4 of these Instructions.

Responsibility for the selection of candidates submitted for the award of the Mother's Glory medal, the timely and high-quality execution of applications for the nomination of mothers of large families for the award lies with the applicant submitting the proposal for the nomination, and the administration of the municipal district (urban district).

7. Establishment of the Ministry of Labor of the Republic of Belarus within a month from the date of receipt from the administration of the municipal district (city district) of an application for the submission of a mother of many children for awarding the medal "Maternal Glory":

checks the submitted documents;

fills out a questionnaire for a mother of many children presented for awarding in the form in accordance with Appendix No. 2 to this Instruction;

sends for consideration by the Ministry of Labor and Social Protection of the Population of the Republic of Bashkortostan a completed application form, a petition from the administration of the municipal district (urban district) with the duly certified copies of the documents specified in subparagraphs "a - c", "h", "k - m" of paragraph 4 of this Instruction.

Copies of the application and documents received from the administration of the municipal district (urban district) remain at the institution of the Ministry of Labor of the Republic of Belarus.

If necessary, the Ministry of Labor and Social Protection of the Population of the Republic of Bashkortostan requests other available documents from the institution of the Ministry of Labor of the Republic of Belarus.

8. The Ministry of Labor and Social Protection of the Population of the Republic of Bashkortostan considers the submitted documents specified in paragraph 7 of these Instructions and submits a proposal for awarding the mother of many children with the Mother's Glory medal to the State Awards Commission under the Head of the Republic of Bashkortostan.

9. The Commission on State Awards under the Head of the Republic of Bashkortostan considers the submitted award materials in statutory terms, evaluates them, prepares materials and presents an opinion on a possible award, prepares draft Decrees of the Head of the Republic of Bashkortostan.
